Oakfield Care & Support
Your local authority will assess the level of support you need and this may be one to one support or shared support, or both. You may require support with personal care and with various other tasks and activities.
The care and support services are provided either by Oakfield Care & Support, or by other providers of your choice.
What does Oakfield Care & Support include?
Our care and support service is person-centered, and tailored to the individual.
Personal Care
One of the areas you may require support is with personal care and developing a morning and evening routine. This may include administering medication.
You will have a detailed support plan. We regularly evaluate your support plan to ensure it remains current. In addition we complete an annual review. It is important to us that you and your family are fully involved in developing your support plan, as it belongs to you. We will also consider any risks and will do all we can to reduce these, so you can live as full a life as you choose. Where there are any decisions to restrict your freedom or choices, we will ensure we do this correctly and in line with the requirements of the Deprivation of Liberty Safeguards under the Mental Capacity Act 2005. We will provide the support you are assessed as needing when you need it which will be provided by our highly trained group of staff.
Daily Living
You may require support with your daily living, including cooking, cleaning, laundry and shopping.
You will have a support worker, who will work with you to develop your skills, wishes, ambitions and social interaction. Information will be circulated to the support team so that everyone is aware of your choices, decisions and wants, as well as what you do not like or would prefer not to take part in. You will be supported by a team of staff, who will work with you and support you within the home and out in the community.
Activities
Your support will include any activities built into your care package. Activities can be in-house or externally in the community.
Some examples of activities our tenants currently take part in include arts and crafts, karaoke, games, walks, accessing day services such as our seed-to-plate project and others, volunteering, gardening, bowling, horse riding, shopping, the cinema, theatre and enjoying lunch or dinner out.
As Oakfield is a community, we hold monthly “Being Heard” meetings. This is where you have another opportunity, along with other people we support, to raise any issues or suggestions you may have. You can tell us what you are happy with and what needs to change.
